The '6.2 million' figure that dominates headlines counts patient pathways, not individual patients — some people are waiting for more than one type of treatment, so the number of people affected is somewhat lower than the headline suggests, though still substantial.

The number that matters more than the total

The total list size gets the headlines, but the more useful number for judging NHS performance is the share of patients waiting longer than 18 weeks against the official target — because the total list naturally grows as more people are referred for treatment in the first place, which isn't necessarily a bad sign on its own.

Where the waits are longest

Waiting times vary substantially by specialty and region — a fact that headline national figures tend to flatten. Elective orthopaedic and ophthalmology waits have generally been among the longest, reflecting both demand and workforce capacity in those specialties specifically.
